IIR Filter Coefficients

I have two equations that I need to put into the following form:

The two equations are:

1. 6960d1214399514-digital-iir-filters-1.jpg

2. 6963d1214399604-digital-iir-filters-3.jpg

Here is an example:

6965d1214399718-digital-iir-filters-eg.jpg

Would anybody like to take a crack at this?

dont know how to write it like above.
((4)(z^-2)+(-8)(z^-1)+4)/((4+w-2w/Q)(z^-2)+(2w-8)(z^-1)+(4+w+2w/Q))
that is my answer to the first question. i used w instead od omega

Thanks for trying.
Sorry, I'm not sure what the correct solution is, but thats not it apparently; my filter becomes very unstable when I use those coefficients.
